Dermal fillers are amongst the most common aesthetic treatments conducted in the UK. They are commonly used to combat the key signs of facial ageing including loss of facial fat, loss of facial volume and the treatment of static wrinkles - wrinkles not associated with facial movements. They can also be used to enhance certain facial features especially the lips and cheek bones. Treatment is usually quick and the results normally immediate. Effects can last between 8 months and 2 years depending on the product used.

How does it work?

There are a variety of dermal fillers available on the market that all have varying degrees of volumising effects and last for a variable amount of time. The most commonly used fillers are hyaluronic acid fillers. Hyaluronic acid is a glycosaminoglycan that is found naturally in abundance in good healthy skin. It has the ability to bind large amounts of water. Hyaluronic acid fillers typically last for around 8 months before they are reabsorbed. Other less commonly used synthetic fillers consist of poly-L-lactic acid, polycaprolactone and calcium hydroxyapatite. These fillers last for longer and have a more rejuvenating effect on the face as a whole. However, there are added risks associated with these fillers and are less easily moulded.

What is it used to treat?

The face has a complex arrangement of pockets of fat within the cheeks. These are well shaped and taut in our younger years. As we age these pockets of fat lose their shape, shrink or enlarge, and slip downwards resulting in the characteristic folds of the cheek and jaw that we develop as we age. Fillers placed in the right areas can help to replace fat that has been lost and make pockets of fat more taut providing more definition and balance to the face that can turn back the clock and make you look younger. For younger clients discrete amounts of filler can be used to enhance lips and cheek bones to help with definition. Fillers can also be used to treat certain deep wrinkles and scars and over time have a rejuvenating effect on facial skin.

Is there scientific evidence to support this treatment?

There is a relatively large amount of scientific data available regarding the effectiveness and safety of dermal fillers. Numerous groups have studied the effects of different filling agents in terms of how long they last, their side effect profile and developed extensive protocols advising regions of the face best suited to specific types of fillers. It is becoming widely recognised that the ageing process in not just about the skin and how it looks but more of a three dimensional process. Hyaluronic acid has developed the largest following owing to its safety profile and the ease with which it can be moulded in the face once applied. There is however, a very definite place for other fillers.

What happens during the treatment in clinic?

During your initial consultation we will listen carefully to your concerns and discuss your expectations of treatment with dermal fillers. We will conduct a thorough three dimensional assessment of your face using the latest 3D scanning cameras. We will work with you to plan where placement of fillers will achieve the best possible result you are looking for whether that be as part of an anti-ageing strategy or for more definition of certain facial features. We administer local anaesthetic nerve blocks if appropriate, to make the procedure as pain free as possible. Following treatment we will repeat our three dimensional assessment and provide a before and after comparison to establish whether you are satisfied with the results and if need be where top ups maybe required.

Are there any side effects to treatment?

Through good technique and counselling we always try to minimise side effects as much as possible and make the procedure as comfortable as possible for you. Side effects include bruising, swelling, redness, and a small subset of clients can develop mild delayed swelling and reactions. With longer lasting fillers it is rarely possible to develop small nodules and swellings called granulomas.

Downtime: Will I have to take time off work? Can I attend social engagements?

The procedure typically lasts around 30 to 40 minutes depending upon the amount of filler used and regions injected. You should be able to return to work immediately and should be able to attend social engagements.

What sort of results can I expect?

The effects of dermal fillers are immediate. We will conduct a three dimensional analysis and demonstrate improvements to you. Hyaluronic acid fillers last for around 8 months and then start to be reabsorbed. With hyaluronic acid fillers you may also notice improvements in the overlying skin. Longer lasting fillers tend to be associated with a slight loss of effect in the first four weeks followed by a gradual increase up to 12 weeks and this lasts for up to 18 months.
